I got a response to my mail to Linear about those FET-diodes.
The most interesting part is the links to manufacturers of low-
leakage diodes.

1) Yes, there should also be two 2N4393's to Vee.
2) The 2N4393's were optional, in cases where low series resistors
AND low leakage was desired. If you can use 5k series resistors
(4.99k is the standard 1% value), then the internal diodes will be
adequate for ESD protection (see the diodes in Figure 1 of the data
sheet).
If you want to use lower value series resistors and don't care much
about DC leakage, then you can just use 1N4148's or BAV99's (dual).
If you want to use low series resistors AND you want low leakage,
then you shold add a low leakage diode. Central Semiconductor makes
some (the CLL3595 and the CMPD3003 for example). Leakage
approximately doubles every 10 degrees C.
